FERPA Agreement
Non-Disclosure Acknowledgment
Please be aware that pursuant to the Family Educational Rights and Privacy Act of 1974 (FERPA) and Florida student confidentiality laws, the University is required to maintain student records in a confidential manner. Based on these laws and the interests of our students, the University, and in turn the University Registrar, take significant measures to protect student confidentiality.
FERPA and the related confidentiality laws permit the disclosure of confidential student information to UNF personnel in only specified situations. One such situation is where the requestor has a “legitimate educational interest” in receiving the information in order to perform his/her assigned UNF job duties. Please be advised that is the only basis on which our office will provide you with requested student records and that
you are not permitted to share this information with any third parties
who do not have a similar legitimate educational interest in reviewing the particular educational records.
This limitation includes others both within and outside the UNF community
including parents and legal guardians
.
In the event a third party requests that you provide him/her confidential student information, please refer the requestor to the University Registrar so that we can address the request. There are a number of technical situations where the Registrar can release information to third parties; however, as the designated custodian of all student educational records, our office should be responding to such requests. As a result, we have adopted this procedure not only for the protection of the students, but also to prevent unlawful disclosure of student records by UNF personnel.
In addition to the above, once you are in custody of confidential student information, you must maintain it in a confidential manner in order to prevent its disclosure and further, you must agree to destroy the student information in a manner that completely protects the confidentiality of the student once you have no further use for it. If you do not have a way to destroy the confidential student information, you may return the information to the University Registrar upon termination of use.
Because of the serious nature of the confidentiality of student records, any requestor who fails to maintain requested student information in accordance with this Acknowledgement, may be reported to his/her supervisor and/or have his/her rights to receive requested information from the University Registrar terminated. If you have any questions or concerns, please contact the University Registrar.
By signing below I acknowledge and agree that I have read and understand the terms and conditions upon which I will be provided confidential student information and further that I will comply with the terms and conditions of this Acknowledgment.
